NECK PAIN – CERVICAL SPINE PAIN Mobilizations With Movement
www.ProPTRehab.com This patient experiences pain in her cervical spine. She expressed concern for being able to turn her head as she has to turn her head while driving. Mobilizations with movements, ala Brian Mulligan, PT procedures can help restore and/or optimize joint function and mobility. Manipulation can be a useful procedure of a variety of joint problems, however, it is often used with everyone who enters one’s facility. Manipulation involves taking a joint to it’s end-range available motion and is performed with a high velocity thrust. If this helps people, it would be advisable to see if taking the joint toward it’s end-range without the thrust will accomplish full pain-free ROM. End-range thrust often results i ‘cavitation.’ Cineradiographs have reported increased joint space which is a form of laxity. While pain may be remediated, the joint is now more susceptible to postural and gravitational forces; hence. If the joint can be moved while it’s in it’s normal physiologic status, then carry-over potential is more likely to occur. Posture and body mechanic instructions combined with patient self treatments can maintain improved function of joints. There is definitely a place for Manipulation, but in my 20+ years I have found little need for it in most patients.
